We Offer Care for several Chronic Conditions



Transportation

We strive to make your life easier by providing transportation (generally within a 10-mile radius of the center). One-way or round-trip transportation is provided from a participant’s home to the our center for an additional fee.


Our van is equipped to handle wheelchairs and walkers as well as ambulatory participants. Scheduling is comprised of personal choice, availability and location.


Many of our participants travel to and from our center via LantaVan, Pennsylvania's “shared ride” transportation program for people requiring specialized service.  Call 610-432-3200 for information on eligibility and registration.  


ABILITY MONITORING AND RESTORATIVE NURSING  

Every participant at SarahCare of the Lehigh Valley is formally assessed on a quarterly basis to evaluate changes in their ability to perform daily life tasks.  The assessment process is led by our Director of Nursing.


Based on these assessments, our staff works with families to ensure a continuum of care for every participant.  


Our Restorative Nursing program is an RN supervised nursing intervention program designed to help participants with assessed needs including:

- Toileting plan or bowel training        - Active or passive range of motion

- Splint or brace assistance                 - Bed mobility

- Walking                                                  - Transfer

- Dressing or grooming                        - Eating or swallowing

- Amputation/prosthesis care              - Communication training

We work with a variety of Funding sources

Standard of excellence

CARF is the only independent accreditation body for adult day care.  Sarahcare subscribes to the CARF standards of care for adult care.

Not only can you come to our SarahCare Center, but we can come to your house anytime of the day or night, providing our same trustworthy and dependant services you have come to rely upon.


Our services allow for our participants to remain socially active during the day, while maintaining a sense of self-sufficiency in their own home.


We provide “wake up”: and “tuck in” services as well as home traditional unskilled home care services.
